I've been using NewPKI (www.newpki.org) which has worked moderately well for me.

Not that I know of, but a quick search on Freshmeat was fruitful.

Web-based: http://freshmeat.net/projects/catool/ [$$$] http://freshmeat.net/projects/ejbca/ [flexible once configured, docs leave much to be desired] http://freshmeat.net/projects/cawh/ [web site has very little info]

Local:
http://freshmeat.net/projects/tinyca/
http://freshmeat.net/projects/xca/
http://freshmeat.net/projects/pyca/ [dead?]

Also, a toolkit for writing your own:
